I. RETIREE HEALTH INSURANCE PLAN:
Approval is requested authorizing renewal of the Medicare eligible healthcare plan benefits and the United Healthcare Medicare Advantage PPO plan. The renewal will become effective January 1, 2017.
II. PHARMACY BENEFIT MANAGEMENT CONTRACT:
Approval is requested authorizing a one-year pharmacy benefit management contract with Pharmavail Benefit Management with services beginning January 1, 2017 through December 31, 2017 with the option to renew for four additional twelve-month periods for services in accordance with the specifications of the contract. The benefit consultant has recommended Pharmavail Benefit Management to provide services for Columbus Consolidated Government employees and dependents.
III. ALLOCATION OF HOME FUNDS FOR AN AFFORDABLE HOMEOWNERSHIP PROGRAM:
Approval is requested
to enter into a HOME Written Agreement, between the Columbus Consolidated Government and Neighborworks Columbus, Inc., to award Home Investment Partnership Program (HOME) funds in the amount of $300,000 to administer an Affordable Homeownership Program. The Affordable Homeownership Program will provide first mortgage assistance, gap subsidies, and down payment assistance to assist a minimum of three (3) individuals or families in Columbus who meet the qualifications of the program.
IV. MIDTOWN REDEVELOPMENT PLAN TAD # 5:
Approval is requested to create tax allocation district # 5 Midtown West, to designate the boundaries of the redevelopment area and tax allocation district, to establish the last known tax increment base and adopt a redevelopment plan for the area, to establish the intent to issue and sell tax allocation bonds as necessary to effectuate the redevelopment of the area and for other purposes.

V. MIDTOWN REDEVELOPMENT PLAN TAD # 6:
Approval is requested to create tax allocation district # 6 Midtown East, to designate the boundaries of the redevelopment area and tax allocation district, to establish the last known tax increment base and adopt a redevelopment plan for the area, to establish the intent to issue and sell tax allocation bonds as necessary to effectuate the redevelopment of the area and for other purposes.
VI. GEORGIA DEPARTMENT OF EDUCATION 21ST CENTURY COMMUNITY LEARNING CENTERS GRANT:

Century Learning Centers grant from the Georgia Department of Education and, if awarded, accept a reimbursement amount valued to total up to approximately $500,000.00 over a five year period or approximately $100,000.00, or as otherwise awarded, with no local match involved, and amend the Multi-Governmental Fund by the amount awarded.

ACCEPTANCE OF DONATIONS IN SUPPORT OF "THE DREAM LIVES," DR. MARTIN LUTHER KING, JR. (MLK) COMMEMORATION EVENT.

Scope of RFB
Provide one (1) stump grinder with tracks and trailer to be used by the Urban Forestry and Beautification Right-of-Way Maintenance Division of the Public Services Department for grinding stumps from City right-of-ways and other City properties.

Columbus Consolidated Government is seeking proposals for a professional engineering firm to evaluate and design repair methods for four (4) slope failure locations in Columbus, GA. The services shall be provided in accordance with Federal standards for FEMA Grants.

Scope of RFB
Provide welding and fabrication services for various facilities and equipment owned by the City. The equipment may be located in buildings, ball fields, watershed lake valves, swimming pools, the Muscogee County Jail, Muscogee County Prison, and vehicles at the Auto Shop. The services will be procured on an “as-needed” basis. This contract will be for two (2) years, with the option to renew for three (3) additional 12-month periods.

Construct the proposed concrete multi-use trail connecting the existing Fall Line Trace and Riverwalk. The proposed trail will be constructed along 10

Security Cameras & Accessories for Buses (Annual Contract) – RFB No. 17-0019
Scope of RFB
Provide security cameras, which will be installed on buses of Metra Transit System. Approximately 0 – 28 cameras will be purchased each year, on an “as needed” basis. The term of this contract shall be for one year, with the option to renew for two (2) additional twelve-month periods.

Macon Road Fire Training Center – RFB No. 17-0020
Scope of RFB
The work consists of, but is not limited to the following major elements: 1) Site Work: To complement Fire Station 9 on Macon Road. Site work to include earth fill, excavations, grading, paving, drainage, utilities, soil erosion control, roadways, concrete pads, etc. 2) Fire Training Tower: Columbus Consolidated Government has purchased a prefabricated 5-story tower to be constructed by others, coordinated by the successful bidder, on the site. General Contractor’s work will include coordination, site prep, concrete on slab work, concrete footings and rebar, concrete work within the 5-story tower, and utility work and connections to Owner provided and installed gas props, etc. 3) Control Room: Concrete slab on grade, load bearing masonry walls with brick veneer, steel roof trusses, metal roofing, approximately 500 sf of new construction for a control room with toilets and shower. Work includes all civil, paving, landscaping, mechanical, electrical and plumbing.

Dodge Charge Police Pursuit Vehicles – RFB No. 17-0022
Scope of RFB
Provide Twenty (20) Dodge Charger Police Pursuit Vehicles. The vehicles will be used by patrol officers of the Columbus Police Department.
