I. FY2014 OFFICE OF JUVENILE JUSTICE AND DELINQUENCY PREVENTION:
Authorization is requested to submit a grant application to the Office of Juvenile Justice Delinquency Prevention for a $244,565 grant with a 25% cash match of $74,855 to enhance Muscogee County Juvenile Drug Court Services under the Chattahoochee Judicial Circuit to include Harris County and if awarded, amend the Multi-Governmental Grant Fund by the amount of the grant award. Harris County has agreed to fund the 25% local cash match for this three year grant period. This Grant will be used to enhance Muscogee County Juvenile Drug Court Services under the Chattahoochee Judicial Circuit to Harris County. An agenda report and resolution are attached.

II. HEALTH AND WELLNESS CENTER CONTRACT:
Authorization is requested for annual renewal of the Health and Wellness Center Vendor, Transform Healthrx of Georgia for one year beginning January 14, 2014 through January 13, 2015 with the option to renew for four additional twelve-month periods, for health care services. An agenda report and resolution are attached.

III. BENEFITS MANAGER CONTRACT - NORTHWESTERN BENEFITS CORPORATION OF GEORGIA:
Authorization is requested for renewal of the annual contract for benefits manager with Northwestern Benefits Corporation of Georgia for one additional year beginning June 1, 2014 through May 31, 2015. An agenda report and resolution are attached.
IV. ACCEPT QUIT CLAIM DEED - 3508 OBERLIN DRIVE:
Authorization is requested
to accept a Quit Claim deed from the State of Georgia, State Properties Commission for a 2.9 acre tract, Parcel 072 008 008, located at 3508 Oberlin Drive, that was utilized by Columbus Technical College as part of the Horticulture Program. The program has been discontinued and the property was declared surplus by the Technical College System of Georgia. The General Assembly and State Properties Commission approved the surplus of property and conveyance of property to the Columbus Consolidated Government.

Scope of RFB
Provide repair/rebuild services for various types of transmissions for Public Works Department – Fleet Maintenance Shop on an “as needed” basis. The contract period will be for three years with the option to renew for two additional twelve-month periods.

Pest Control Services (Annual Contract) – RFB No. 14-0043
Scope of RFB
Provide pest control services for one-hundred (100) City-owned buildings. Contract requirements include the control of pests including: mites, ticks, spiders, rodent, roaches, ants, reptiles, bats, fleas and etc. The contract period will be for two (2) years with the option to renew for three (3) additional twelve-month periods, if agreeable to both parties.

On-Call Services for Installation or Replacement of Fencing Located Throughout Columbus, Georgia (Annual Contract) – RFP No. 15-0001
Scope of RFP
Install or replace fencing, to include but not limited to: chain link, wrought iron, post and rail, aluminum, and vinyl fence systems throughout Columbus-Muscogee County, on an as-needed basis. The City intends to contract with at least three vendors. For each project, the Engineering Department will obtain written quotes from said vendors. The term of the contract shall be for two years, with the option to renew for three (3) additional twelve-month periods.
